5th Grade Discipline Plan:At the beginning of each quarter, each student will receive a SuperStar card that has five stars on it. In order to attend the SuperStar party at the end of the quarter, Students must present their cards as tickets.
Students are responsible for keeping track of their tickets. Students receive hole-punches on one of their five stars for: missing homework assignments (only once per day, even if more than one assignment is missing that day) or inappropriate behavior After five holes are punched on the card, students will lose their cards for the sixth missing homework or behavior issue. Students who still have their cards at the end of the quarter have earned the SuperStar party. In addition to potentially losing the privilege of attending the SuperStar party, students will sit at a silent lunch table each time they receive a hole-punch on their card. Eachtime a student receives a hole-punch for inappropriate behavior, a “Think Sheet” will be filled out by the student, signed by the student and teacher, and sent home. Students must return the sheet with a parent signature the next day. The teacher and parents are free to add comments to the sheets. If the students have filled out three "Think Sheets" for the same behavior, they will be refered to the office. Students will be referred directly to the office for incidents of deliberate misconduct. In this case, there may not be a “Think Sheet.” Students automatically lose their cards if they receive an office referreal. |
